If you are making a claim on your insurance policy, then it can be assumed that you have either suffered a serious medical condition (critical illness) or a loved one has passed away.
In either case, the process for filing a claim with ivari is clear, and straightforward. This article will detail how to make a claim for a critical illness policy or a life insurance policy.
Making a Critical Illness claim
To submit a critical illness policy claim, you will first have to complete a Claimant’s Statement (CL1477). As well, your doctor will need to complete the Critical Illness Attending Physician’s Statement (CL1476).

Making a life insurance claim
In 2023, ivari processed and paid 99.5% of our life insurance claims. We are here to make the process as easy as possible for you during what we know is a difficult time.
What you need to know
Not every claim follows the same process, so the first thing you should do is contact our Claims Department, toll free, at 1-855-806-5057 or by email at [email protected]. A Claims Adjudicator will look at your specific claim and give you detailed instructions about which forms to complete and what is required to process your claim. If your claim falls under any of the following situations, you may want to review the information here first and then contact the claims department:
The policy is under 2 years old
For all claims on policies under two years old, there is a “contestability period.” This simply means that the claim will have to go through a routine investigation process to look at the insured’s health history prior to the date of the policy being issued. This is an industry-wide practice that can take time to complete, but the good news is that most of the time the claim is paid. The policy is more than 10 years old and is for less than $100,000
Policies that fit this description and have had no changes made in the past 10 years, may qualify for express handling when it comes time to make a claim. The ivari claims department will let you know if your claim fits within the express handling criteria.
Foreign deaths
Any death that occurs outside of Canada and the United States is considered a foreign death claim. These types of claims may require additional investigation due to the high incidence of international insurance fraud.
In addition to the standard claim requirements, there are also a number of foreign death requirements and your claims adjudicator will advise you of specific items needed.
Accidental deaths
Given the exclusions in the Accidental Death Riders, in addition to the standard claim requirements, we often require:
- the coroner’s report and police reports. We may have to investigate how the accident happened before we can approve these claims
